I had a tubal pregnancy before. But it was finished
without any deffects to my tubes. Them I moved to live
in Canada. It is very hard to see a specialist here before
several months. The last period delayed for 2 months with
pain in my sides and my abdomen was larger than usual.
Then when the pereiod come my abdomen go back to normal state.
I had tests for all the hermones in addition to ultrasoud and
everything was fine. Now my period delayed again for 12 days
and and home pregnancy test is negative.
What do you thing the cause for this period interruption ?
What should I do?

Dear Sister,

You seem to be suffering from irregular ovulation. This happens a lot to women who had a negative experience during their pregnancy. If all your hormones are normal, I would suggest that you receive combined contraceptive pills for three months (pills that contain both estrogen and progesterone) for twenty one days every month. This will help your periods to become regular and usually your ovulation will become normal again.

I’m begging you to take my complaint seriously. Your reply will be my last chance to live. And excuse me if I’ll write the details, I can’t tell any one. I’m suffering a lot and I’m dying now. I’m 25 years old, white female. In 9/2001, when I was 23 years old, I joined aerobic classes to lose weight.(I had a very good, strong body built). By then I was my height was 157cm,my weight was 65kg. I was exhausting myself In the exercises and was following a diet. Once, I ate a lot, so I through up in the bathroom intentionally. I didn’t imagine that I’d repeat this behavior. I found it easy way to lose weight. I kept doing this every time I eat. Then I became real and typical bulimic. I lost 20kgs within less than 3 months. I suffered a lot of symptoms: Dizzy, exhausted, skinny, couldn’t tolerate cold, with pale, puffy & exhausted face. I used to have terrible feeling in my head that I can’t explain it. As if my head solidify, and as if I’m fighting just to be aware. Something happened to me twice: I was talking normally with a friend, and suddenly I felt terribly dizzy, with hyper breathing and tackycardia. So, I sat down immediately because if I wouldn’t, I knew I was about to faint. All that happened in the gym. Once I stopped eating for 3 days to punish myself. I was only drinking water. The forth day I returned to eat and through up. After a week, I stopped eating for four days again. The fifth day I was bingeing and through up. Then, after few days, although I was completely fine, when I finished playing in an aerobic class, and while I was talking to the instructor, suddenly without no introductions, I felt that feeling in my head again, the instructor asked me if I am fine. I answered: I don’t think so, then I fainted. I was semi unconscious, then I had terrible convulsions. A typical picture for what happens to a frog when you inject it with a strychnine. I felt then I was dying. It was a chaotic situation for me and lasted for 3-5 minutes. When the ambulance came they checked my blood pressure & it was normal. Physical examination, blood and urine tests were completely normal. But the way I look like was as if I am mommy. I didn’t tell I was bulimic. I left the gym, but kept being a typical bulimic with the complaints I mentioned. I kept being bulimic for a year. I stopped for about 8 months, I gained 15 kg from those I lost before. then returned back to that awful behavior in worse form. I have been being bulimic again for 10 months till now. But this time, I wasn’t bingeing. I was eating like a normal person eats. But I through up every thing I eat. I may vomit up to 5 times aday. In fact whatever I eat, I through it up even if it is just salad. In the last month I became both anorexic and bulimic. I drink coffee and tea a lot. I suffer the following: Dizzy most of the time, with icy hands and foot, dry skin, yellow teeth, mental fluzziness, confusion, anxiety, chills, pale puffy face, lack of energy, constipation (I take any laxative contain Bisacodyl occasionally for the constipation), hair loss, sores in my throat and yellow mucous, flatulence and distention if eat anything, oliguria and very dark urine. When I started to have terrible stomach pain, palpitations, and frequent attacks of tackycardia, shortness of breath and chills(I feel as if I’m going to dye during these attacks which most often occur during the middle of night, but may also occur suddenly during the day). I was afraid to have a heart failure. I went to the doctor. Again complete physical examination, blood and urine tests were completely normal. I didn’t tell then I was bulimic. The symptoms became worse, and once I was fasting just one day, and while I was talking to my friend I felt very dizzy, severe tackycardia and shortness of breath. So I sat down immediately. I went to the doctor again. And again physical examination was completely normal. The doctor told me I am in a big dilution. All what I need is to eat. Again I didn’t tell then I was bulimic. After a week I went to him when I became in a chaotic situation. While I was waiting to enter, I felt very dizzy, shaking , with icy hands to the extremities, I felt my head solidify, a numbness sensation all over my body. I was dizzy, confused, I felt palpitations and shortness of breath. I couldn’t enter. I think they called him to me. He took me in the clinic. Again physical examination was completely normal. And I’m sure he thought I was pretending. He told me it’s a dilution. I told him I’m a bulimic. He said he can’t help me because it’s not his specialty. He suggested to see a psychiatric. I know he is right but I will never do this. He asked me to tell my parents, but I’ll never do. He asked me to take a decision whether I agree to go to a psychiatric or he don’t want to see me in his clinic again. He is waiting for me this Wednesday but I’ll not go. Please, my life is horrible. I can’t live like that any more. Every time I take a real decision to quit, I find myself become worse and worse. The symptoms are worse, the attacks are more frequent. I began to harm my self. I stopped eating today and I intend not to eat again. I will not go to a psychiatric. I’m convincing myself now that my symptoms are just dilutions as he said. The worse is I’m about to commit suicide. Please I’m crazy. I’m losing myself. Please tell me if my symptoms are delusions as he said. What’s my end exactly???!!! I’m not myself. I’m 45kg right now. All those who used to know me told me I disappeared. The doctor told me: your body seems normal to me. Please help me. You are my last chance. If you will ignore my story,…I know my!! Please reply now!!!!!!!! PHYSICAL EXAMINATION ON CHEST(HEART BEATS, PULSE, X-RAY) ,BLOOD & URINE TESTS ARE ABSOLUTELY NORMAL. ARE MY SYMPTOMS & COMPLAINTS ARE REALLY DELLUSIONS????? I'm very moved by your question. You are definitely suffering a lot. Your major problem is psychological. You need urgent psychological support. You have to know that you are truly diseased and it's not just something you are imagining. I would recommend very strongly that you consult a reproductive psychiatrist. You will find help at Ain Shams University Psychiatry Center at Al-Abassiyyah where you can consult Professor Gehan Al-Nahhas who is specialized in your type of case. A piece of good news is that your disease is curable! And it will go away once you receive the correct help.

The hope is there, but I need more information in order to give you proper advice. The problem seems to be in the motility of the sperms and this is correctable. On the other hand, there might be some difficulties on your side that are making things take a little bit more time. So please send us information about your hormones, your ovulation, your tubes and whether you had tried any assisted reproductive techniques.

My wife is almost 8 months pregnent. Our doctor told us that we should consider cesarian birth due to some reason which i didnt really understand.

May you please explain the reasons that may lead to such births and my wife is really scared from the idea that she will undergo surgery, it would be really kind if you layed out how are these surguries done??

Assalamu alaikum.

Dear Brother,

Cesarian delivery is something that doctors refer to in order to ensure safety of both the mother and the fetus, so if there are causes in the mother that would make a normal delivery risky, as for example a contracted pelvis, an abnormal cervix, or an abnormal outlet for delivery, the doctor might see it's safer to have a CS.

On the other hand, if the baby is premature or lying in a position where delivery would be difficult, again the doctor might think it's safer to have a CS.

I would recommend that you sit with your doctor and talk to him frankly about your concerns, and ask him to explain in detail why he is taking that decision. At the end I want to reassure you that a CS today is not a dangerous surgery.

An ultrasound at four months is not very dependable in confirming the fetal gender. You would have to reconfirm the gender in a following ultrasound; maybe at the beginning of the fifth month of pregnancy.

PLease could you help me I am 28 years old , I had been having severe period pains and heavy/prolonged period for the past 2 years or so and this april had a scan and was told i had a cyst in right ovary 13/8cm. So i had a keyhole surgery in which i was diagnosed with severe endometriosis. the right ovary was drained at 15cm and the right tube was distorted and left side also has multiple ovarian cysts but left that as it is becasue she said i had dense adhesions on the left side,ovary,tube and the small bowel is stuck and my chances to concive are threw ivf. I dont have any children as yet as im not married. At that time the surgeon said i would need a further keyhole which may convert into a laprotomy also to release adhesielysis. I have been on zoladex since then, i have had 5 injections now and should be having this operation next wek on short notice sometime.

I wanted to know firstly if there is any alternative medication/therapy from surgery as im not realy keen to have this. And if this is the only option is there anything i can request teh consultant to do to restore fertility. I did ask her as ivf is my only chances , can you clear the tube but she says it will only get blocekd again in couple months but i feel that as im geting married in febraury/march is that a wise option cause it couldnt get any worse with the tubes so is there any harm trying tubal surgery etc as im having the surgery anyway or is that best left aloone.

please reply a.s.a.p as the surgery would be next week so your advice would be really appreicated at this time.

Dear Sister,

Endometriosis is a complex condition. On one side it prevents pregnancy, on the other side pregnancy would make it go away. So I think it is OK to go into surgery with the hope of trying to correct the adhesions surrounding your ovaries and tubes as much as possible the adhesions.

You would then be given medications and you would be given time to try and get pregnant naturally. But eventually if you don't get pregnant, you will have to undergo assisted reproduction (IVF).

Keep your hopes high because today the results of IVF are improving.

Bellies are really different during pregnancy and no two women would look alike even if they were of the same duration in pregnancy. So just keep on your antenatal care and don't think a lot about the way you look because as long as you're pregnant you will look wonderful!

Twin pregnancy can be diagnosed as early as four weeks by ultrasound which would show two sacs. By eight weeks, the ultrasound should be able to distinguish clear fetal heart beats and at 16 weeks the doctor could tell you the fetal sex.

My question is about my exterior genital organs, more precisely concerning the libia minora( small lips ), they are very long than they should be and that make me feel very,very bad.

So, I know that I can have a solution with surgery. Please let me know about that , of whether it demands to stay a long time in a hospital, about any harm that surgery can cause and if possible about how much money this operation may need.

I know that my case is not rare and that there are other women with the samething but really I can not bear seeing them that long , especially that I will be engaged quite soon and I want to avoid any nuisance with my futur partner insha Allah.

Dear Sister,

Your condition could be solved through a very simple yet precise operation called "labioplasty". It differs from the practice of cutting off the labia in the fact that it preserves the nerves and the blood vessels inside the labia and only the extra skin is removed. The skin is refashioned in order to look fine and preserve the function of the labia.

Cost will depend upon where you do it. It's a day surgery, and you could go home a couple of hours after the procedure. You would need to do it immediately after your period, and at least two or three months before you get married.

Yes. A lot of women would get spotting if they miss a single pill. To these women the advice is "take the pill on time!"

A well woman examination and a pap smear depend upon the woman's age, sexual activity, and history of risk factors. So it must be fashioned in a way that would be both effective and convenient for every woman.

Probably you would need a confirmatory ultrasound. Four months is a little bit early to be very sure, but boys usually appear clearly on ultrasound. So hopefully you would be given by God what would make you satisfied.

Sex is a very complex behavior. There is no reason to avert from having sexual relations with your wife during pregnancy unless there is a threat to pregnancy anticipated by her physician.

Pregnant women do not have less desire, arousal, or orgasm than non-pregnant women. The problem is they might have negative attitudes towards having sex during pregnancy and this usually originates from misbeliefs that sex might harm their babies. Some women don't feel well enough to have sex because of the tiring symptoms of pregnancy, or because they don't really fulfill their pleasure during sex. If everything is taken care of, and a proper atmosphere is created, a pregnant woman will function sexually in a very normal and efficient way.

I need to remind you that sexual behavior isn't a one-way ticket. You have to make sure that the relationship will happen in an atmosphere of mutual interest, consent and pleasure.

Hope you are feeling warm! This is our first question from Alaska.

Now there are five major points that need to be fulfilled to make a decision about cesarian section.

1. How the baby is lying in the uterus. If the baby is lying longitudinally, not oblique or transverse, then we can proceed to #2.

2. What part of the baby presents first? If it's the head, so we can proceed to #3.

3. The mother's pelvis. If it's of adequate size, we proceed to #4.

4. The contractions. Are they strong enough and happening at regular intervals? Go to #5.

5. The cervix. Is it dilating properly and in proper time?

These are the major five steps for deciding whether a woman would deliver normally or by cesarian section. But I need to remind you that there are a lot of other factors that have to be considered like your general health, your blood pressure, presence of medical disorders like diabetes or kidney disease, and lastly the condition of the baby whether it would withstand the stress of labor.

So as you see, sometimes the decision for cesarian section can be straightforward and taken before delivery even begins, and sometimes the doctor has to wait to see whether the contractions and the cervix would dilate properly during labor. It's not an easy job!!

You are putting on too much weight. You need to pay more attention to what you are eating so as to keep fit and healthy. Try to increase your vegetable and fruit intake, keep your milk skimmed and avoid completely fast foods, trash foods and useless calories.

The normal weight gain during pregnancy would be around 9 to 14 kilos according to your starting weight. So in your case, you began pregnancy at 60 kilos, you should have stopped at 70.

I became pregnant first time when I was 24 years old.I breastfeed my first child for almost 1 and 1/2 year to 2 year long.My breast became saggy after my first delivery & breast feeding.I feel very sorry for this.I told i was 24 when it became like that.Generally this happens to all or it happened in my case.It happened because of pregnancy or because of breast feeding.I feel very sorry when I see this.what's the reason of this?I see some women after breast feed also their breast look firm. Dear Ayesa,

Breast changes during pregnancy and lactation are totally normal. They depend very much upon your weight gain during that period because the breast is composed majorly of fat tissue. So if you gain a lot of weight your breasts will become heavy and this will lead to stretching of the connective tissue fiber support of the breast. Unfortunately this never goes back to normal, so women must take care not to gain excess weight during pregnancy and lactation, have their breasts supported all the time, and try to lose any excess weight as soon as possible.

Your condition can be corrected surgically if it is causing you a lot of distress.

I'm 23 yrs old,have never been married. For 1 week now I've been suffering from
pain in the vulva with increasing sevrity I also noticed multiple small lesions on the vulva and perineum with enlarged tender inguinal lymph nodes. I took analgesics and antibiotics(without visiting a doctor) and I felt partial relief but the lesions are still there.

Any advice?

I would advise very strongly that you consult either a gynecologist or a dermatologist as soon as possible. You might be suffering from anything from a viral infection up to skin allergy. Only the expert eye and sometimes the lab would be able to tell the difference.

Sometimes tubal ligation is reversible according to the technique of the ligation itself. The easiest to reverse would be a "band ligation". An impossible technique to reverse would be a "fimbreactomy" in which the fimbreal end of the tube is excised.

Dear Waleed. Your second question is a very delicate one. Our Islamic scholars have great disagreement around that time, but in my personal opinion once you diagnose pregnancy, you know for sure that a new human being has been created whether he deserves the legal status of a human being is not something for us to consider, because God has created and is not the right of anybody to put claim the knowledge of when creation actually began.

Your third question, a tough one. I know that it is still under consideration at Dar al-Ifta in Egypt and hopefully we will know the status soon.

My question is.. i often notice one or two drops of blood in my underwaer in the early morning when i wake up, even when im not having my period.

I wouldnt really prefer going to a doctor if its nothing alarming. thats because muslim female doctors arent easy to find here.

but if it's a serious condition, i'll be compelled to go. i'll leave that decission to you.

I think that you have to see a doctor. It might be nothing, just a simple infection or inflammation, but on the other hand we have to exclude if it is a serious condition. If you have to see a non-Muslim doctor that is not a problem and the only thing is you have to make sure that he/she is a trust-worthy doctor.

this question is about circumcision and I need some answers please.
my 10 months old son has been circumcised 4 days ago & had just the bandages removed. I found that there are 2 "rings"; one just around the the head of the penis, and the other which is rotten and swollen behind the head. does this sound normal or do I need to take my son to a doctor?

I don't really know what you mean by "rings". If you mean plastic rings that are used for bloodless circumcision, this could be normal as the mechanism involves sloughing of part of the skin and this would make it appear rotten. On the other hand, if you mean that there is a skin ring that seems rotten, then this is a serious problem and you have to consult the surgeon who performed the circumcision as soon as possible.

Dear Aneesa,

If you're talking about pregnancy and the effect of naproxen on the fetus, I think you have to know that we don't have enough evidence in the literature about the long-term safety of this drug. On the other hand, if you mean the effect of the drug on adults, I think the drug has been studied quite enough to ensure its safety.

Dear Deena,

Candida could be a real problem. You have to avoid a lot of predisposing factors. The most important is wetness in the genital area. You have to keep yourself as dry as possible. Try to choose natural fabrics for your clothes, as these will allow your skin to perspire normally. You have to disinfect your underwears and I would recommend using a chlorine solution made of 9 parts water and one part chlorox, and have your underwear soaked in it for 10-15 minutes. You can use the same solution to disinfect the toilettery so that you don't reinfect yourself. You have to wash your hands properly with an antiseptic solution like savlon every time you go to the bathroom. You have to make sure that while washing yourself water doesn't come from the direction of your anus towards your genitals because the anus is the major reservoir of candida.

Saying that, now you need to take oral candidal therapy. I would recommend Itraconazole tablets to be repeated after every period for three months, daily wash with boric acid solution 2%, which has to be prepared by a pharmacist, and you can use any of the antifungal creams on the genital area.

Mabrook (congratulations) for your marriage!

Kids are the gift of Allah. He awards them to whomever He desires. Hopefully Allah will give you what you wish.

You need to be sure that you are ovulating properly. If you have regular periods and your hormones (FSH and LH) are within normal ranges, then your chances of getting pregnant are great.

It would thus be recommended that you have your hormones (FSH and LH) checked.

Assalamu alaikum once again. This extra piece of information is very important. Now you need to make sure that your cervix is completely healthy and that you are not suffering from any illnesses before planning to get pregnant. I think that you don't have any barriers to pregnancy, but your health is of utmost importance, and if pregnancy would affect it negatively, then you should consider contraception.

I am a mother of three Alhamdullilah. my youngest child is 7 months old. after the delivery i have not been able to loose weight, as compared to my first child, where i lost weight very quickly. I am breast feeding my baby, and have not had my periods yet. i have tried excercising and deiting, with no positive results. I had the same problem after my second delivery. Nothing would help me loose weight while i was feeding my child. It was only after the breast feeding period was over that i went on atkin's deit and lost 10 kg. I am stuck with the same problem again. kindly tell me what is the reason for this, and also is it safe for my to gp on an atkins deit during the feeding period, i.e., will it affect my baby?

Dear Sister,

Breast feeding will make a woman hungry and that is why women gain weight during lactation, simply because they are eating a lot.

There are specific diets for breast-feeding mothers, and these should be followed up under the supervision of a dietician. I would advise very much that you seek help from a specialist.

My 10 month son has been circumcised 4 days ago and we have just removed the bandages. Behind the ring of skin, which the removed bandages have now exposed, there is some skin that is white and damaged. Will this heal and is this to be expected? We contacted the surgeon who did the circumcision by phone as he lives 2 hours away and he said don't worry.

You need not to worry. Just keep the baby's skin clean all the time. Use a mild anti-septic solution like savlon and an anti-septic or anti-biotic cream. The skin will heal wonderfully inshallah.

Most anti-helmenthic drugs are dangerous and should not be used during pregnancy, except for strong purgatives. Other drugs are cytotoxic and will negatively affect the baby.

I would refer you to a dermatologist. Psoriasis in the vulval area is an annoying condition, but therapies are present and I think a specialist would be able to help you.

Dear Ali,

The problem with circumcision in girls is that it is practiced in so many ways. Sometimes the clitoris and the labia are excised and this is not only against religion, but also it causes a lot of problems ranging from sexual problems to major health conditions like bleeding, infections, infertility resulting from the infections, and urinary splashing (due to removal of the labia) and incontinence. Some women will encounter difficulty during delivery if they had extensive fibrosis following the circumcision. So, if circumcision (by the way it's a wrong word) is to be done only a small piece of skin covering the clitoris is to be removed and that is only done if a physician sees that the piece of skin could cause irritation or discomfort to the girl.

The correct term to be used in describing the practice of cutting the clitoris or the labia would be female genital cutting.

Dear Rabab,

If your eggs reach 16mm then we have great hopes. You need to enroll into an assisted reproduction program at any of the specialized centers. They will put down a plan and inshallah you will get pregnant.
