Answer It really matters, especially after marriage because you may have premature ejaculation and loss of satisfaction at the time of ejaculation ( Orgasm). It is up to you to stay away from this habit and do something more meaningful.

assalamu alaikum warahmatullah
i am married 4 years. 8 months before i took my wife for check up. the doctor told she is still virgin. my wife body is very cold. she is not able to strech her legs and is very uncomfortable streching and always closes her legs.
1. i am fourty years old. i have a penis after erection 4 inches.
3. when i intercourse it becomes soft and the lenght decreases.
4. the doctor told that i need to do intercourse in the hole but i am not able to do it. and suggested that i should consult a doctor.
i went to a doctor i had a penile doppler scan and after that he gave me a medicine i dont remember the name. still i am not able to use it as i am sick.

Answer
First of all, I recommend that you take sildenafil pills to have good erection (before taking this medication, you must consult a doctor in your area to see if this medication is suitable for you). This should take care of the virginity issue.

After that you should do some exams like semen analysis to rule out any infection which could affect the erection. You also must do lab tests (FBS, Cholostrol, triglecaride, LH, prolactine, testosterone) to rule out any other diseases like diabetes or vascular diseases or any other hormonal disturbances. Also you have to do an ultrasound on the abdomen and prostate and to show all these exams to a urologist in your area.

As for the frigidity of your wife, this might very well be psychological. I would strongly recommend foreplay. I would very strongly recommend counseling or visiting a gynecologist or a specialist in your area.

If you are experiencing this urge day and night as well, it is necessary for you to do some exams, like urine-analysis and ultrasound to the abdomen and full urinary bladder and another film after voiding. The results should be seen by a urologist. However, if your problem is during the day only, it could be psychological or emotional problem more than organic (pathological.)

I have been diagnosed with non obstructive azoospermia. Doctors told me only chance to have a child is to go through a testicle surgery to extract sperm. Is there any other options?

Answer
The best you can do is to follow the instructions of your doctor, but if you want to try another option, you have to do a test called "vasogram" to see the the exact place of obstruction and then to go a microscopic surgery to do an anastomosis of the vase difference (vasostomy or vasoepididymostomy.)

The success ratio for such operations is not higher than 30% in the best centers in the world. I wish you luck.

I get urinary tract infections frequently and i take ciprofloxacin, but it does not go away.

Why i am susceptable to urinary tract infections and what is a good cure for it?

Answer
The best thing for you is to stop all medications for at least five days. Then, you can do urine analysis and culture and to take the suitable antibiotic. You also have to do an ultrasound to the kidneys and the urinary bladder to rule out any factors that may contribute to the infection like stones or any congenital defects in the urinary tract. Finally, you have to take a good amount of fluids every day which helps in treatment.

I have been married for three years and my husband has a problem in that he ejaculates too quickly, before intercourse.

What can be done about this problem???

Answer
That is what we call premature ejaculation which could be pathological or psychological. We have to rule out the pathological aspect of this problem by doing a semen analysis and culture and an ultrasound to the prostate (to rule out chronic prostatitis.) If the test results are normal, we can try anti-depressant drugs like Seroxat tablets 20 mg once daily for one month at least. However, if it is chronic prostatitis, he should take a course of antibiotics for at least 28 days.

Hello,

I suffer from kidney stones which cause me severe pain and colic. What is the most effective treatment for this and what foods should I avoid inorder not to increase the problem?

Answer
For me to recommend foods you should take, I must know what type of stone you have. For this reason we have to do an analysis to the stone to know its makeup.

We also have to search the pathological reason for the stone (if possible) by doing lab tests like "parathormone test". As for treatment, it all depends an the size and location of the stone and if there are any changes in the kidneys. Nowadays we have other options of treatment rather than surgery like uritroscopy, extracorporal shock wave lythtripsy and percutaneous nephrolyhotomy.

Generally speaking, we recommend a lot of fluids every day and a regular lab tests and ultrasound and IVU (Intravenous Urography) if needed.

Answer
Depending on what kind of "disease" you are talking about. If you mean infection, drinking a good amount of fluids during the day is highly recommended. If you mean sexually transmitted diseases, we recommend using a condom. In general there are some diseases that you can prevent by taking care in advance. There are other diseases like cancer, for example, that it is difficult to prevent.

No its is not true!!! But I recommend urinating after intercourse in general because usually it is difficult to urinate immediately after intercourse because the bladder neck during and 30 minutes after ejaculation will be partially closed and it opens after that. So urination will be difficult and painful (dysuria).

Answer
Urinary tract infections by themselves, even if recurrent, do not affect pregnancy. But recurrent vagainitis or any other infection to the reproductive system in women could possibly affect or even prevent pregnancy.

Answer
First the quality of the semen can be measured by a test called semen analysis in any lab. So you can know the quality of the sperm concerning count, motility and morphology of the sperm.

I do not know what you mean by self treatment. You can not count the sperms in the sample yourself, thus you must go to a lab.

As for the frequency of intercourse, it all depends on you as well as your partner. Your psychological wellbeing has a lot to do with it as well. Your age may very well play a great factor in deciding the frequency.

Answer
What you are having is normal in general. It is simply a prostatic fluid but if you want to be sure you do not have a problem, you have to do a semen culture and an ultrasound to the prostate to rule out chronic prostatitis.

You normally would need to make wudu again but not ghusul. However, we refer your question to the local mosque for a better answer to this question.

Since I am a doctor, not a mufti, I will give you my personal view. I do not believe that medicine or science in general should be monopolized be either of the sexes. As long as one performs his or her duties professionally the sky is the limit. Again, this is my personal point of view on this matter.

NO. Condoms do not pose any problem to either fertility or the semen quality. You can use it to plan your family, pregnancy, or other related matters.

I am 65 yrs old and have an enlarged prostate. I take pepon which has pumpkin seed. I am worried that this is not sufficient and the enlargement might get worse?

Can you please tell me what to do and what are teh dangers of having an enlarged prostate?


Answer
Prostatic enlargement is in close relation of your quality of life. The biggest danger is having a retention of urine ( not able to urinate at all). In this case you need a surgery. For prostatic enlargement I recommend some other medications like Fenestride which decreases the size of the prostate in long-term use. There are other medications called alpha-blockers that opens the neck of the bladder and the prostatic urithera so you can void easier.

However, 85% of patients with prostatic enlargement are treated medically, not surgically. Only 15% or so need an operation called TUR-Prostate. (Trans-Urithral Resection of prostate) and not open surgery.

Hypospadias is an abnormal opening of the external meatus (external urithral opening) which is not in its normal place in the tip of the glands of the penis. There are degrees of hypospedias which could be coronal, sucoronal, mid-shaft, or scrotal opening. Treatment is by doing plastic surgery to the urethra to correct it. Surgery should only be bone before school age (5 yrs).

It appears that your husband does not have any pathological dysfunction. I recommend that he consult with his psychiatrist whether he could stop or change this antidepressant. Meanwhile, he could try sildenafil to see if it makes any difference.

I have varicose veins on my right testes and i need to do an operation, could you please tell me what will happen in this operation and if this will affect intercourse after it is done?

Answer
Varicocele is an enlargement of the internal spermatic vein (widening) more than 3 mm on average. It affects mainly the activity of the sperm and partially the count of the sperm. In the operation we used to make a small incision under general anesthesia on the right side of the abdomen high or low to the inguinal region and to ligate the spermatic vein or veins.

The operation does not affect the intercourse at all.
