Question What causes infertility in men?
Answer The causes of infertility in men are very numerous, the cause may be hormonal due to defects in the pituitary gland, the hypothalamus, or a thyroid gland disorder. It could also be due to a defect in testicular function which could be genetic, traumatic, inflammatory or neoplastic in nature or due to toxins and drugs, smoking and drug abuse, heat or radiations. There are also post testicular causes, such as obstructive or erectile dysfunctions or ejaculatory disorders.

Question How is infertility tested?
Answer Male infertility is tested when at least one year of continuous unprotected marital relations do not lead to pregnancy. First a clinical assessment is done by a specialized physician and this is followed by the analysis of the seminal fluid. Further investigations are done according to the results of the previous 2 steps.

Answer Yes, they could have a negative effect. It is therefore advised for those exposed to hot tubs for long periods to receive a semen analysis every 6-12 months.

Question What are the differences between male infertility, impotence, erectile dysfunction, and premature ejaculation?
Answer Male infertility is the inability to impregnate one’s wife after one year of continuous unprotected marital relations.
Impotence and erectile dysfunctions are synonyms and both mean the inability of a man to obtain and/or to maintain an adequate erection to complete sexual relations. Whereas, premature ejaculation is rapid persistent ejaculation within 2 minutes after penetration.

Answer Yes, it is possible through microsurgical techniques, but the results are dependent on the technique of vasectomy used as well as the length of time between the vasectomy and the reversal.

Question What are the causes of lost sexual desire in men?
Answer Many factors may cause inhibited sexual desire in men. Psychogenic causes such as depression, anxiety, and phobias stand for many cases. Interpersonal factors between a man and his wife are also important.

Among the important organic causes are hormonal disturbances such as low testosterone (hypogonadism), hyperprolactinemia and hypothyroidism. Also, systemic diseases such as renal and hepatic failures and terminal illnesses can all cause low sexual desire in men.

Question If Impotancy strikes a Muslim, What Halaal treatments are available?

What if you have a wife, How can you explain to her about impotancy and does she have the right under Islaam to leave you & how can you satisfy her in other ways which is Halaal?

What if the Impotancy is incurable, What treatments are available to have children without intercourse?

Having children in life is very important, Is there other methods of having them if impotant?

If not married, can you marry with this illness?

What has Allah and Mohammad said concerning impotancy and is there any Quran chapters and verses which can help towards a cure?

Answer Impotence is a curable disease, and there are many available lines of treatment which are all halal. This could be medical by means of drugs, or local injections of vasodilators. It could also be surgical by means of arterial/venous surgeries or penile implants. Sex therapy by behavioral modifications can help many cases. There are no cases of impotence that are not curable or amenable to correction.

Regarding fertility issues in men with impotence, it is possible to have children by means of assisted reproduction, but treating the impotence itself is preferred. In unmarried men, impotence is usually due to psychological factors, and its treatment is very easy. The rest of your questions need to be submitted to a scholar rather than a physician.

Question Why most of the cases come from ejaculatory disorders?
Does Islam hint to overcome this menace?
Answer Ejaculatory disorders especially premature ejaculation are the most common male sexual disorders reported by patients. This may be due to too much concern regarding female satisfaction, the negative attitudes of women in our culture towards sex, the wide spread practice of female circumcision in our community, the relative lack of sexual education in both men and women or sexual misconceptions due to availability of pornographic materials on the internet and satellite TV stations.

Question Do biking and extreme exercise affect male fertility? How?
Answer No, these activities do not affect male fertility, however, concomitant use of large doses of anabolic steroids has a detrimental effect on male fertility due to suppression of the hypothalamus and pituitary glands.

Question What are the cures and treatments available for premature ejaculation?
Answer Curing premature ejaculation is possible if there is a definite cause that is curable such as prostatitis, neuropathy and multiple sclerosis or if it is drug induced. However, most cases are idiopathic (have no underlying cause) and there are multiple lines of treatment available. There are oral drugs that are available, the best of which are SSRIs (Selective serotonin reuptake inhibitors) and tricyclic antidepressants. There are also topical treatments by using local anesthetic creams or sprays and augmentation by the injection of fillers. In addition to behavioral therapy by certain sexual exercises applied by a trained physician.
